Job summary

Qualifications

• Specialized Diploma graduate of an accredited school of nursing required
• Knowledge of patient- and family-centered practice and collaboration with care partners in planning care
• Cultural awareness and ability to connect with diverse backgrounds
• Effective communication to transmit, receive, and interpret information with patients and families
• Knowledge of patient assessment and care planning
• Knowledge of patient and designated care partner education on condition, treatment and follow-up
• Knowledge of medication administration and prescribing to continue treatment
• Knowledge of discharge planning and arranging follow-up care
• Knowledge of assessment and diagnosis and clinical judgement
