The Registered Nurse, Telehealth is responsible for providing telephonic triage, directional patient care advice for general and specific illnesses, health-related issues, client counseling, patient advocacy, health education, and referral and resource management to ChenMed patients and their families. The incumbent provides remote clinical advice and assessments within license and as technology allows. He/She collaborates with primary caregivers and others on the interdisciplinary care plan team to provide a team approach of care.

This role is a full-time opportunity.

Schedule: Monday - Friday 1630-0030 EST

Essential Job Duties/Responsibilities

1. Connects with patients via phone or video call, interviews and questions patients to collect health history, and uses a computer system to record and store comprehensive and focused data relating to the health needs of patients and families.
2. Provides health assessment and treatment solutions, monitors patient health and at-home care, aids in emergency scenarios, and promotes patient wellness.
3. Monitors a patient’s oxygen levels, heart rate, respiration, blood glucose, and other assessment measures, based on available technology.
4. Uses video chatting to identify patient’s symptoms and conditions, analyzes data to determine appropriate health maintenance, and identifies outcomes for the patient and family.
5. Collaborates with on-call PCP as needed to support expected clinical outcomes, implementing appropriate protocols.
6. Evaluates and documents progress toward anticipated outcomes, ensuring achievement of optimal patient outcomes through telemedicine, and documents interventions clearly.
7. Enhances the quality and effectiveness of telehealth practices and professional nursing through successful utilization and outcome improvements.
8. Utilizes resources to plan and provide safe, effective, and fiscally responsible services.
9. Performs other duties as assigned and modified at the manager’s discretion.

Education And Experience Criteria

1. Associate Degree in Nursing required; Bachelor’s preferred
2. Multistate Nurse Licensure required
3. Licenses for Michigan and Illinois required within 90 days; additional licenses as needed
4. Spanish fluency required
5. Minimum 2 years of nursing experience in ER or triage setting
6. BLS certification from AHA or ARC required; 3+ years of acute clinical nursing preferred
7. 1+ year of virtual care experience preferred
