Job summary

Senior Lifestyle's Lincolnwood Place in Illinois is seeking a Registered Nurse for the night shift in a skilled nursing setting. The role covers medication supervision, admissions assessments, and care planning while coordinating with the nursing leadership and cross-functional teams.

You will provide direct patient care, respond to emergencies, document thoroughly, and educate residents and staff. A current IL RN license and CPR certification are required.

Qualifications

• Associate degree in nursing or equivalent required.
• Current RN license in the state of practice (Illinois).
• CPR certification and basic computer skills expected.

Responsibilities

• Medication supervision/administration in line with regulations and company guidelines.
• May assist with admissions assessments and documentation.
• Develop and maintain acute and long-term care/service plans.
• Track clinical data under Nursing Director guidance (wounds, infection control).
• Handle discharges, assessments, and clinical documentation as needed.
• Provide health-related consultation within the community.
• Notify physicians/families of health changes with proper documentation.
• Respond promptly to emergencies; communicate with staff.
• Maintain resident confidentiality and participate in Safety/Disaster plans.
• Lead educational meetings for residents and staff.

• Responsible for medication supervision/administration in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.


• May be responsible for the admissions of new residents to include all admission assessments and documentation.


• Responsible for completing acute and long-term care plans or service plans.


• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the Director of Nursing.


• May be responsible for discharges, assessments, and clinical documentation as indicated.


• Provide general consultation regarding health-related concerns within the community.


• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.


• Respond quickly to all emergencies.


• Maintain close communication with all departments and staff members.


• Maintain confidentiality of residents’ and prospective residents’ information.


• Understand role in the Safety & Disaster Plan.


• Provide educational meetings for residents and staff.

Level of Formal Education: An Associate’s Degree or equivalent from a two year college or technical school: or Two years related experience and/or training equivalent combination of education and applicable experience.

Special Certifications: Must have a Registered Nurse’s License current, with no judgments against it, in the state in which he/she is practicing and must be CPR certified
