Job summary

Interim HealthCare is hiring a Home Health Registered Nurse (RN) in Ohio. You’ll deliver high-quality home-based patient care, with flexible scheduling and control over your days. Join a team that values autonomy, compassionate care and work-life balance.

Responsibilities include patient assessments, care planning, education for patients and families, and coordinating discharges. Must have an accredited nursing degree and an active RN license in Ohio/Compact States, CPR, and 1+ year of nursing

Qualifications

• Graduate of an accredited nursing program with an active RN license in Ohio or Compact States.
• Minimum of 1 year of nursing experience, ideally in acute care and/or home health care.
• CPR certification.
• Knowledge of or orientation to state and federal home health regulations.
• Clinical proficiency in nursing, strong communication skills and compassion.

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases, often after hospital discharge.
• Collaborate with physicians and oversee LPNs, CNAs and aides delivering patient care.
• Assess patients, document progress and report changes to the physician.
• Educate patients on plan of care, medications, therapy, diet and exercise.
• Coach family members on the plan of care, medications and home safety.
• Coordinate discharge from services to ensure goals are met.

As a Home Health Registered Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facility
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care
