Job summary

Interim HealthCare of Wichita is seeking a Home Health Registered Nurse (RN) to deliver compassionate, patient-centered care in patients' homes in Wichita, Kansas. You can set your own schedule, balancing work and life while delivering high-quality nursing services.

As part of a dedicated team, you’ll coordinate with physicians, supervise LPNs and aides, assess patients, educate families, and support discharge planning.

Qualifications

• Graduate of an accredited nursing program and active RN license in Kansas.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency and strong communication skills.

Responsibilities

• Provide home-based care to patients in Wichita, KS.
• Coordinate with physicians and supervise LPNs, CNAs, and aides.
• Assess patients, document progress, and report changes to physicians.
• Educate patients and families on plans of care, medications, therapy, and home safety.
• Coach family members on the plan of care, medications, and safety at home.
• Assist with discharge planning and service coordination.

As a Home Health Registered Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after discharge from a hospital, rehab or nursing facility
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care
• Examine patients, do assessments, document progress and report changes to their physician
• Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
• Coach family members on the patient’s plan of care, medication and home safety
• Ensure goals are met and coordinate discharge from services
