Polaris Healthcare and Rehabilitation Center, a 100-bed facility in Milford, Delaware, is seeking a compassionate and experienced RN Unit Manager to join our team. You will oversee the unit, apply the nursing process to assess, plan, implement, and evaluate resident care, and ensure high-quality, compassionate services for our residents.

The ideal candidate is a licensed RN with at least two years of nursing experience, familiar with Federal, State, and local long-term care requirements, and

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate resident care.
• Assess signs and symptoms indicating changes in the patient's condition.
• Document the patient's plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions.
• Perform interventions according to identified priorities, plan of care, hospital policies, and patient care outcome standards.
• Revise the plan of care according to evaluation, changes in the medical plan of care, and effective/ineffective nursing interventions.
• Use clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor, including establishing priorities.

As a RN Unit Manager, you will be responsible for managing the unit, utilizing the nursing process to assess, plan, implement and evaluate resident care, and ensuring that our residents receive high-quality, compassionate care.
