Position Description
Responsible for ensuring that care delivery is consistent with the missions and policies of the company and in accordance with accepted standards of practice, state and federal regulations and licensing requirements. Responsible for the overall operations, integration, coordination, and direction of nursing care during assigned shift; includes mentoring and educating nursing personnel, working with physicians and other medical professionals.
Principal Responsibilities
- Ensures delivery of high-quality, positive clinical outcomes, and resident/family satisfaction during supervisory shift.
- Serves as a resource to all staff within the facility regarding the quality of services provided.
- Provides direct resident care when required; demonstrates the ability to administer medications and treatment timely and according to facility policy; demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act.
- Effectively manages nursing personnel on shift to limit use of overtime and agency personnel.
- Assesses the work performance of nursing staff as it relates to unit standards of care and goals.
- Conducts regular staff meetings on each shift and provides updates to DON.
- Collaborates with other departments to provide timely effective care consistent with individual needs, choices, and preferences.
- Assumes nursing administrative responsibilities of the center for weekend and holiday coverage as requested.
- Stays and works beyond scheduled shift if needed to meet staffing requirements and resident needs.
- Ensures resident care assignments are consistent with staff competencies, scope of practice, and resident needs.
- Ensures timely and accurate compliance of the MDS process and that it reflects the resident’s plan of care.
- Ensures Point Click Care (PCC) is utilized according to company process.
- Ensures physician orders are followed as prescribed.
- Oversees medication management to ensure adequate supplies and proper handling per policy.
- Ensures prompt notification of significant health changes to physician and family or responsible party.
- Ensures communication between licensed nursing staff and CNAs during and between shifts.
- Participates in shift-to-shift communication and reports incidents, accidents, adverse events, and grievances promptly and thoroughly.
- Investigates resident/family complaints and takes appropriate actions.
- Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
- Notifies other departments of resident changes and conducts rounds to evaluate care effectiveness.
- Ensures adequate PPE supplies and evaluates care interventions for effectiveness, identifying problems and trends.
- Performs other duties as assigned.
Direct Care Responsibilities
- Conducts daily nursing functions per regulations; provides direct resident care when needed.
- Charts progress notes accurately reflecting care and resident responses.
- Reports changes in condition promptly; follows skin management protocols.
- Orders and reports diagnostic tests timely.
- Counts narcotics at shift changes.
- Follows pharmacy policies for medication handling.
- Follows procedures for restraint reduction.
- Assists with meals, monitors nutritional status, and checks external food items for safety and compliance.
- Documents exceptions and participates in investigations related to abuse or incidents.
Special Nursing Care Responsibilities
- Demonstrates knowledge and ability to work with various specialized procedures and equipment, including NG tubes, colostomy, Foley catheters, IVs, tracheostomy, and ventilators.
Human Resource Responsibilities
- Implements staffing strategies, enforces policies, and conducts staff education and coaching.
General Responsibilities
- Attends training, maintains certifications, and adheres to work schedules and policies.
Customer Service Responsibilities
- Models excellent customer service, treats residents and staff with respect, and explains procedures clearly.
Residents Rights
- Upholds and promotes residents’ rights, maintains confidentiality, and responds to concerns promptly.
Safety and Sanitation
- Follows safety policies, reports hazards, and participates in drills and safety procedures.
Staff Development
- Participates in ongoing education and communicates policies and regulations to staff.
Qualifications
- Graduate of an approved RN program and licensed in the state.
- Minimum 2 years of nursing experience preferred.
- Current BLS/CPR certification.
Travel
- Local travel for training and workshops.
