Job summary

Community Rehabilitation Hospital West, part of Lifepoint Health, is hiring a Registered Nurse (RN) for a full-time role with day and night shifts. This 36-hour work week features 12-hour shifts from 7:00 to 7:30, offering opportunities to provide compassionate, patient-centered care.

You will perform nursing interventions, document care, monitor patient conditions, support daily living activities, and collaborate with care teams to coordinate transfers and education.

Responsibilities

• Provides direct patient care using the nursing process in accordance with scope and standards of practice.
• Performs plan of care interventions, including medication administration and other medical treatments.
• Documents patient care provided.
• Monitors, records, and communicates patient condition to care team, physician, patient, and family.
• Assists patients with activities of daily living (ADLs) including hygiene, elimination, nutrition, and ambulation.
• Collaborates across disciplines to coordinate patient care, transfers, discharges, and referrals.
• Evaluates learning needs of patient and/or family and provides education appropriate to age, culture, and condition.
• Advocates for the physical and emotional well-being of the patient.

Position Summary

Provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.

• Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
• Documents patient care given.
• Monitors, records, and communicate patient condition as appropriate to care team, physician, patient, and family.
• Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
• Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
• Works as an advocate for the physical and emotional well-being of the patient.

Qualifications

• Graduation from an accredited Bachelor of Science in Nursing, Associate Degree in Nursing or Nursing Diploma program.
• Current state licensure as Registered Nurse.
• CPR/BCLS certification. ACLS preferred.
• Excellent oral and written communication and interpersonal skills.
