Job summary

Elara Caring is hiring an experienced Registered Nurse Home Health to join our field team in the Houston area. You will assess patients, develop and update care plans, deliver nursing care per physician orders, and educate families on safety and care at home.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, Associate or Baccalaureate).
• At least 1 year of nursing experience in a clinical setting.
• Home Health experience preferred but not required.
• Ability to lift up to 50 pounds and assist with patient positioning and moving equipment.
• Valid driver’s license or reliable transportation for travel between patient homes.

Responsibilities

• Assess patients to determine their care needs and develop care plans.
• Complete initial and ongoing patient assessments and document changes.
• Provide nursing care per plan of care and physician orders.
• Educate patients and families on care, safety and disease management.
• Monitor patient conditions and report changes to the care team.
• Ensure privacy and HIPAA compliance, and contribute to ongoing quality of care.

As a Registered Nurse Home Health, you’ll contribute to our success in the following ways:

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations including Southwest Houston, Medical Center, Missouri City, Richmond, Rosenburg, and Sugar Land.
