Job summary

Adaptive Home Health, LLC in Detroit seeks a part-time Registered Nurse (RN) to deliver in-home patient care and drive high-quality outcomes. This PRN role includes field-based visits with flexible scheduling and mileage reimbursement.

The RN will perform comprehensive assessments, document accurately using OASIS, and collaborate with physicians to update the care plan while supervising supporting staff to ensure exceptional patient care.

Qualifications

• Active Michigan RN license.
• Reliable transportation and valid driver's license.
• Home health / OASIS experience strongly preferred.

Responsibilities

• Conduct in-home assessments with comprehensive documentation (OASIS, SOC/recert/ROC, discharge).
• Provide direct, high-acuity patient care with focus on wound care.
• Build and manage the plan of care with physicians; supervise LPNs and HHAs.

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

1. Carry a high-quality caseload: Ensure visit quality, with OASIS accuracy that holds up to review.

2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you'll own

• Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).

• Direct, high-acuity patient care, with a focus on wound care

• Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.

• Being the face of Adaptive to patients, families, and the referral sources who feed your pod.
