Job summary

Adaptive Home Health, LLC is seeking a Registered Nurse (RN) Case Manager to deliver high-quality home health care in Detroit. This field-based role partners with physicians to build and manage plans of care, while leveraging our AI-native platform to streamline charting and documentation.

You will own comprehensive in-home assessments, oversee wound care cases, and supervise LPNs and HHAs. Michigan RN licensure and reliable transportation are required, with home health/OASIS experience

• Home health / OASIS experience strongly preferred.

• Comp: $64.10 per point

• Mileage in addition to productivity comp

• Schedule: Full-time Monday–Friday + some weekends (as needed)

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

1. Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review.

2. Own your patients\' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.
