Job summary

Adaptive Home Health, LLC seeks a dedicated Registered Nurse (RN) Case Manager for field-based assignments in Detroit. You will carry a high-quality caseload, perform in-home assessments, and document care using OASIS, SOC, and ROC.

Your clinical expertise drives patient outcomes and supports the pod’s success. In this role you supervise LPNs and HHAs, collaborate with physicians, and act as the face of Adaptive to patients and referral partners.

Qualifications

• Active Michigan RN license required.
• Home health / OASIS experience preferred.
• Reliable transportation, valid driver's license, auto insurance.
• Willingness to work field-based visits and Detroit-area assignments.

Responsibilities

• Perform comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Deliver direct, high-acuity patient care, including wound care.
• Develop and manage the plan of care with physicians; supervise LPNs and HHAs.
• Represent Adaptive to patients, families, and referral sources.

• Mileage in addition to productivity comp

• Schedule: Full-time Monday–Friday + some weekends (as needed)

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

1. Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review.

2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you'll own
