Mind & Mobility seeks a compassionate Registered Nurse (RN) to provide in-home care for clients in the Palm Harbor, FL area. The RN will assess patient health needs, supervise caregivers, and collaborate with families to promote safety and well-being in a private duty home health setting.

The ideal candidate has strong clinical judgment and excellent communication skills, with flexibility to travel up to 75% and manage a dynamic home health schedule.

Qualificações

• Active RN license in the state of practice.
• Experience in home health, private duty, or outpatient nursing preferred.
• Strong clinical assessment and patient care skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage a flexible schedule.
• Reliable transportation and a valid driver's license.
• Proficiency with electronic medical records (EMR) and basic computer skills.
• CPR and other relevant certifications as required.
• Willingness to travel up to 75% of the time.

Responsabilidades

• Assess patients' health status and needs in the home setting.
• Develop and implement individualized care plans in collaboration with patients, families, and caregivers.
• Educate patients and families on disease management, health maintenance, and safety.
• Communicate patient progress, concerns, and changes in condition to supervisors and other care team members.
• Maintain accurate and timely documentation of care in compliance with company policies and regulations.
• Promote a safe, comfortable, and supportive environment for patients in their homes.
• Conduct caregiver evaluations and assess caregivers' skills to ensure quality of care.
• Perform supervisory visits with caregivers to observe care delivery, provide guidance, and ensure compliance with care plans.

Position Summary:
