Job summary

Senior Care Solutions is seeking a PRN Registered Nurse (RN) to provide skilled in-home care across Florida. The role requires compassionate patient-centered care, strong clinical judgment, and adherence to Medicare regulations.

You will perform SOC/ROC, recertifications, wound care, meds administration, and education for patients and caregivers, traveling across the Pasco/Pinellas area with flexible scheduling. Florida RN license and BLS are required.

Qualifications

• Current Florida RN license.
• 1+ year nursing experience; home health preferred.
• OASIS experience preferred.
• BLS/CPR certification current.
• Valid Florida driver license with reliable transportation.

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, Evaluation, and Discharge visits.
• Complete patient assessments and document OASIS when applicable.
• Develop, implement, and update plans of care.
• Administer medications, wound care, and other skilled nursing tasks.
• Educate patients and caregivers on disease management and safety.

Position Summary

The PRN Registered Nurse is responsible for providing skilled nursing care to patients in their homes in accordance with physician orders, agency policies, and Medicare regulations. This position requires excellent clinical judgment, strong assessment skills, and timely, accurate documentation.

Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, Evaluation, and Discharge visits.
• Complete comprehensive patient assessments, including OASIS documentation when applicable.
• Develop, implement, and update individualized plans of care.
• Administer medications, treatments, wound care, injections, and other skilled nursing services.
• Educate patients and caregivers on disease management, medications, safety, and available resources.
• Monitor patient progress and communicate changes in condition to physicians and the interdisciplinary team.
• Collaborate with Physical Therapists, Occupational Therapists, Speech Therapists, Medical Social Workers, and Home Health Aides to coordinate quality patient care.
• Ensure documentation is accurate, complete, and submitted within agency timeframes.
• Maintain compliance with Medicare Conditions of Participation, state and federal regulations, and agency policies.
• Participate in case conferences and communicate effectively with the clinical team.
• Travel throughout the assigned coverage area to provide patient care.

Qualifications

• Current, active Florida Registered Nurse (RN) license.
• Minimum of one (1) year of nursing experience required.
• Home Health and Medicare experience preferred.
• OASIS experience preferred.
• Current BLS/CPR Certification.
• Valid Florida driver's license, reliable transportation, and current auto insurance.
• Excellent assessment, communication, and organizational skills.
• Ability to work independently and manage a flexible schedule.
• Experience with WellSky/Kinnser is a plus.
