Wythe County Community Hospital is seeking a Registered Nurse (RN) for the Post Acute Care, Skilled Nursing unit. This full-time night position (7p–7a) includes rotating weekends and a supportive team focused on safe, patient-centered care.

You will perform patient assessments, plan care, document medical records, contribute to the interdisciplinary care plan, and participate in quality improvement activities to drive outcomes.

Qualifications

• Associate's Degree required; Bachelor's Degree preferred.
• Basic Life Support (BLS) certification is required within 30 days of hire.
• Active Virginia RN license (single-state) or compact license.

Responsabilités

• Accurately performs patient assessments and identifies patient needs
• Identifies and initiates appropriate nursing interventions
• Provides care appropriate to condition and age of the patient
• Performs timely and appropriate documentation relating to medical necessity in the medical record
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient
• Performs timely and accurate QI assessments

Skilled Nursing on an 8 bed Unit, with supportive nurse-to-patient ratio with a supportive manager that prioritizes patient safety and positive outcomes. Opportunities for learning/professional development and clinical ladder program. Flexibility in schedule that allows for continuing education or balance between work and personal life.

How you'll contribute

A Registered Nurse (RN) who excels in this role:

What we're looking for

Applicants should have an active Virginia RN license (single-state) or compact license. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins.

• Basic Life Support (BLS) certification is required within 30 days of hire

Hourly Range: $29.79 - $40.21
