Job summary

UP Health System - Portage is seeking a Clinic RN to deliver comprehensive patient care in a dynamic team. The role supports medication administration, documentation, and patient education while coordinating with physicians and others to ensure safe transfers and outcomes.

A Michigan RN license and an associate degree are required, with BSN preferred for growth within Lifepoint Health. The position offers full-time hours, a Monday–Friday day schedule, and a supportive environment focused on

• Active Michigan RN license or eligibility.

• Strong clinical and communication skills.

Responsibilities

• Performs plan of care interventions, including medication administration and other treatments.
• Documents patient care provided.
• Monitors and communicates patient condition to the care team, physician, and family.
• Collaborates across disciplines to coordinate care, transfers, and discharges.
• Provides education to patients and families appropriate to age and condition.
• Acts as advocate for patient physical and emotional well-being.
• Delegates care activities to qualified staff.
• Performs prior authorizations for tests, procedures, and medications as required.

How You'll Contribute

• Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
• Documents patient care given.
• Monitors, records, and communicates patient condition as appropriate to care team, physician, nursing home, patient, and family.
• Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
• Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
• Works as an advocate for the physical and emotional well-being of the patient.
• Delegates patient care activities to appropriately qualified staff.
• Performs prior authorizations on tests, procedures, medications, etc. that are required by the patient's insurance.
• Performs other job duties as assigned.

What We're Looking For

Applicants should have a current Michigan RN license (or attain within 90 days of hire) and possess an associate degree from an accredited nursing school (bachelor's degree preferred). Additional requirements include:

• Basic Life Support certification is required (available upon hire)
• Annual Influenza Vaccination or Declination Required
