THRYVE Healthcare in Crestwood, IL is seeking a compassionate Registered Nurse (RN) to deliver high-quality care to residents in our skilled nursing facility. The RN will provide total care on the assigned unit during PM shift, supervise staff, and ensure adherence to facility policies.

Qualifications

• Graduation from an accredited nursing education program.
• Active Registered Nurse (RN) license in the state of Illinois.
• No history of abuse convictions.
• Strong interpersonal and communication skills.
• Ability to follow facility policies and support quality care.
• Must be able to work effectively with residents of varying ages and medical needs.
• Must meet physical job requirements, including occasional lifting, bending, and standing.

Responsibilities

• Provide total care for residents on assigned unit during scheduled shift.
• Collaborate with allied health team members to develop and implement individualized care plans.
• Administer prescribed medications and treatments and evaluate effectiveness.
• Maintain an environment that supports residents’ physical, emotional, and social well-being.
• Educate residents and families about care plans, treatments, and procedures.
• Monitor team performance and support staff development, evaluations, and corrective actions as needed.
• Assign and oversee nursing duties to ensure tasks are completed appropriately and efficiently.
• Stay current on facility procedures and participate in in-service and continuing education.
• Identify and respond to significant changes in residents’ conditions.
• Accurately document care provided and changes in condition per facility guidelines.
• Act as a charge nurse and serve as a resource for staff, offering clinical support.
• Follow all facility and departmental safety protocols.
