Qualifications

• CT RN licensure is required.
• Experience in home care and skilled nursing visits.
• BLS, ACLS, or CPR certification.
• Minimum of one year demonstrated RN experience.
• Ability to explain physician's instructions to patients and families.
• Ability to multi-task with strong organizational skills.
• Valid driver’s license or non-driver photo ID; drivers preferred.

Responsibilities

• Assess client needs to develop a plan of care.
• Collaborate with management on complex cases.
• Coordinate and supervise the plan of care to ensure services meet the plan.
• Monitor patient condition and outcome of care.
• Oversee care provided by LPNs and paraprofessionals.
• Coordinate safe discharge from service.
• Communicate with patient’s MD and family as needed.
• Assist patients with activities of daily living.
• Document services in the internal system.
• Transport equipment and travel to patient homes or facilities.

Alvita Care is seeking experienced Registered Nurse RNs for per diem role in Connecticut (Fairfield, New Haven, Litchfield County). Alvita Care is seeking experienced, CT-licensed Registered Nurses (RNs) to provide private duty nursing services and Skilled Nursing Visits. Ideal candidates are reliable, compassionate, and willing to deliver consistent, one-on-one care.

DETAILS:

• Per Diem Shifts from 6hrs, 8hrs to 12hrs. Day and Night Shifts Available

QUALIFICATIONS:

• CT RN licensure
• Experienced in home care and skilled nursing visits.
• BLS, ACLS, or CPR certification.
• Minimum of One (1) year demonstrated experience as an RN
• Ability to put patients at ease and explain physician's instructions.
• Ability to multi-task with excellent organization skills.
• Experience working directly with people from diverse racial, ethnic and socioeconomic backgrounds.
• Knowledge of medical terminology.
• Team player and ability to work independently
• Valid driver's license or Non-Driver photo ID card

ESSENTIAL DUTIES & RESPONSIBILITIES:

• Assessments needs of the clients to develop a plan of care
• Consult with Management on complex/unique cases
• Provide a plan of care with support of the Clinical/Medical/appropriate multidisciplinary members to ensure the services follow the plan of care by assessment, planning, coordination, counseling, teaching and supervision to meet needs of the clients/patients/.
• Monitor/Assess patients/clients' condition, effects of the care plan,
• Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
• Manages, plans, coordinates and executes the patient's safe discharge from service.
• Ensures that physician's orders, discharge summary and communication to team members, patient and family members adhere to company and regulatory requirements.
• Communicates and consults with patient's MD with all manners when needed, manage and coordinate all aspects of assigned caseload.
• Assists patients with activities of daily living
• Documents content and process of all services provided in internal system properly.
• Transport/utilize medical equipment to and from clients/patients' home/facilities. Travel to patient/clients' homes and/or other facilities.
• Perform other related duties and participates in special project, as assigned.
