Job summary

Alvita Care is seeking experienced Registered Nurses (RNs) licensed in CT to provide private-duty home health visits across Connecticut, including Fairfield, New Haven, and Litchfield County. Positions are per diem or part-time, with day and night shifts available and a focus on reliable, compassionate, one-on-one care in the home.

Qualified RNs have CT licensure, BLS/ACLS/CPR certification, and at least one year of RN experience. Travel to patient homes is required; drivers are preferred.

Qualifications

• CT RN licensure is required.
• BLS, ACLS, or CPR certification is required.
• Minimum of one year RN experience.
• Ability to explain physician's instructions to patients.
• Ability to multi-task with strong organization.
• Experience working with diverse backgrounds.
• Valid driver’s license or non-driver photo ID; drivers preferred.

Responsibilities

• Assess client needs to develop a plan of care.
• Consult with management on complex cases.
• Develop a care plan with the multidisciplinary team.
• Ensure services follow the care plan through assessment and coordination.
• Monitor patient condition and effects of the care plan.
• Oversee care provided by LPNs and paraprofessionals.
• Coordinate safe discharge from service.
• Document services in the internal system accurately.
• Travel to patient homes for visits.
