Job summary

Alvita Care is seeking experienced Registered Nurses (RNs) for per diem home health roles across Connecticut, including Fairfield, New Haven, and Litchfield County. RNs provide private duty nursing and skilled visits in patients' homes, delivering compassionate, one-on-one care.

Qualifications

• CT RN licensure is required.
• Experience in home care and skilled nursing visits.
• BLS, ACLS, or CPR certification.
• Minimum of one year demonstrated RN experience.
• Ability to explain physician's instructions to patients and families.
• Ability to multi-task with strong organizational skills.
• Valid driver’s license or non-driver photo ID; drivers preferred.

Responsibilities

• Assess client needs to develop a plan of care.
• Collaborate with management on complex cases.
• Coordinate and supervise the plan of care to ensure services meet the plan.
• Monitor patient condition and outcome of care.
• Oversee care provided by LPNs and paraprofessionals.
• Coordinate safe discharge from service.
• Communicate with patient’s MD and family as needed.
• Assist patients with activities of daily living.
• Document services in the internal system.
• Transport equipment and travel to patient homes or facilities.
