Alvita Care seeks an experienced, Per Diem Registered Nurse (RN) to provide 1:1 in-home care across Connecticut. Shifts range from 6 to 12 hours with day and night options, and roles can be part-time or fit around your current schedule.

You will assess client needs, develop care plans with clinical support, and ensure services follow the plan. CT RN licensure and 1+ year RN experience required, with BLS/ACLS/CPR; a valid CT driver’s license is required. Bilingual a plus.

Qualifications

• Ability to explain physician instructions clearly.
• Ability to multi-task with strong organizational skills.
• Experience serving diverse populations.
• Valid CT driver license or CT Non-Driver ID.

Responsabilités

DETAILS:

• We have clients throughout CT requiringshifts from 6hrs - 8hrs to 12hrs.

QUALIFICATIONS:

• CT RN licensure.
• Minimum of One (1) year demonstrated experience as an RN
• BLS, ACLS, or CPR certification
• Ability to put patients at ease and explain physician's instructions.
• Ability to multi-task with excellent organization skills.
• Experience working directly with people from diverse racial, ethnic and
  socioeconomic backgrounds.
• Knowledge of medical terminology.
• Team player and ability to work independently
• Valid driver's license or CT Non-Driver photo ID card, required
• Drivers preferred!
• Bilingual a plus!

ESSENTIAL DUTIES & RESPONSIBILITIES:

• · Assessments needs of the clients to develop a plan of care
• · Consult with Management on complex/unique cases
• · Provide a plan of care with support of the Clinical/Medical/appropriate
  multidisciplinary members to ensure the services follow the plan of care by assessment, planning, coordination, counseling, teaching and supervision to meet needs of the clients'/patients/.
• · Monitor/Assess patients'/clients' condition, effects of the care plan,
• · Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
• · Manages, plans, coordinates and executes the patient's safe discharge from service.
• · Ensures that physician's orders, discharge summary and communication to team members, patient and family members adhere to company and
  regulatory requirements.
• · Communicates and consults with patient's MD with all manners when needed, manage and coordinate all aspects of assigned caseload.
• · Assists patients with activities of daily living
• · Documents content and process of all services provided in internal system properly.
• · Transport/utilize medical equipment to and from clients'/patients' home/facilities. Travel to patient/clients' homes and/or other facilities.
• · Perform other related duties and participates in special project, as assigned.

As we expand our circle of care, we are seeking top-notch caregivers to service our Connecticut needs. We have private-duty one-on-one care cases available as well as facility-based care cases.
All applicants are subject to a phone screening to determine compatibility and overall availability of accommodations for both the caregiver and the agency. Please note that cases with Alvita Care are only available to caregivers who successfully complete the onboarding process.
