Job summary

Gentle Care Home Services is seeking compassionate RNs for one-on-one home health visits with children who have medical needs in their homes across Georgia. Assignments vary by availability and location.

You will work with a supportive clinical team, follow each patient’s plan of care, and document care accurately. Active GA RN license and CPR are required; pediatric/home health experience is preferred.

Qualifications

• Active, unrestricted Georgia RN license.
• Current CPR certification.
• Ability to meet background and health screening requirements.

Responsibilities

• Provide one-on-one skilled nursing care in the patient's home.
• Follow the patient's individualized plan of care.
• Perform nursing assessments and monitor changes in condition.
• Administer medications and prescribed treatments.
• Care may include G‑tube, tracheostomy, ventilator, seizure care per plan and competencies.
• Complete accurate and timely documentation.
• Communicate with families and the Gentle Care clinical team.
• Provide patient and family education and support.
