Qualifications

• Active Georgia RN license.
• Current CPR certification.
• Background and health screening requirements.
• Pediatric or home health experience preferred.

Responsibilities

• Provide one-on-one skilled nursing care in the patient’s home.
• Follow the patient’s individualized plan of care.
• Perform nursing assessments and monitor changes in condition.
• Administer medications and prescribed treatments.
• Provide G-tube, tracheostomy, ventilator, seizure and other skilled care as applicable.
• Complete accurate and timely documentation.
• Communicate with families and the Gentle Care clinical team.
• Provide patient and family education and support.
