Centric Home Health and Hospice is seeking an experienced Registered Nurse to join our hospice weekend on-call team in Norman, OK. The role focuses on after-hours patient visits and weekend coverage, delivering compassionate, high-quality care in patients' homes.

The ideal candidate holds an RN license, CPR, and at least two years of clinical experience (hospice preferred), with reliable transportation and strong communication skills to coordinate with physicians and families.

Qualifications

• Graduate of an accredited nursing program (Diploma, Associate, or Bachelor’s).
• Current Registered Nurse (RN) state license.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.
• Strong communication and interpersonal skills.
• Two years of clinical experience required; hospice experience preferred.

Responsibilities

• Conduct initial and ongoing comprehensive patient assessments ensuring hospice eligibility criteria is met.
• Develop, implement, and revise individualized plans of care.
• Educate patients and families on disease processes, self-care, nutrition, and end‑of‑life needs.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Provide clinical direction to Hospice Aides and LPNs/LVNs.
• Maintain accurate, timely documentation of care and progress.
• Participate in on‑call rotation and respond to patient needs promptly.
• Follow infection control and safety procedures.
• Uphold agency policies and professional standards.

• Availability required: 5 pm on Friday through 8 am on Monday.
