Job summary

Centric Home Health and Hospice is seeking a dedicated Hospice Registered Nurse (RN) to provide compassionate in-home hospice care across the Norman area. You will perform patient assessments, coordinate plans of care, and work with physicians and an interdisciplinary team to meet patient needs.

You’ll educate patients and families about disease processes, end‑of‑life care, and self-care, while ensuring accurate documentation and adherence to infection control and agency policies.

Qualifications

• Graduate of an accredited nursing program (Diploma/ADN/Bachelor).
• Current RN license (in-state).
• CPR certification required.
• Valid driver’s license with reliable transportation.

Responsibilities

• Perform initial and ongoing patient assessments to confirm hospice eligibility.
• Develop and revise individualized plans of care.
• Educate patients and families on disease processes, self-care, nutrition, and end-of-life needs.
• Coordinate care with physicians, caregivers, and interdisciplinary team members.
• Provide clinical direction to Hospice Aides and LPNs/LVNs.
• Maintain accurate, timely documentation of care and progress.
• Participate in on-call rotation and respond to patient needs.
• Follow infection control and safety procedures.
• Uphold agency policies and professional standards.

Overview

Minimum Requirements

• Graduate of an accredited nursing program (Diploma, Associate, or Bachelor’s).
• Current Registered Nurse (RN) state license.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance
• Strong communication and interpersonal skills.
• Two years of clinical experience required; hospice experience preferred.

Work Environment

• Care provided in patient homes under varying conditions.
• Exposure to infectious diseases, blood, and bodily fluids possible.
• Requires local travel and ability to work a flexible schedule.
• May involve prolonged standing, walking, and lifting up to 50 lbs.
