Job summary

Complete Care at Green Knoll is seeking an RN-MDS Coordinator to lead MDS assessments in a long-term care setting. You will collaborate with the nursing and administrative teams to ensure accurate regulatory compliance and effective care planning.

You will guide staff through MDS processes, stay current with regulations, and contribute to billing integrity and ongoing quality improvement. This role offers leadership opportunities and a supportive team environment.

Qualifications

• Associates or Bachelor's degree in Nursing.
• Active, unencumbered RN license.
• 2 years proven experience as an MDS Coordinator in long-term care.
• Experience with MDS assessments and regulatory compliance.
• LPN with MDS experience may be considered.

Responsibilities

• Conduct and coordinate the MDS assessments for residents.
• Collaborate with interdisciplinary teams for comprehensive assessments.
• Ensure accuracy and completeness of MDS assessments for care planning.
• Stay updated on changes in MDS regulations and implement adjustments.
• Participate in care planning meetings and resident care planning.
• Support billing and reimbursement processes with nursing staff.
• Provide education and training to staff on MDS processes.

What You’ll Do:

Registered Nurse-RN-MDS Coordinator Qualifications and Responsibilities:

• Associates or Bachelor's degree in Nursing
• Active, unencumbered New Jersey RN license
• Previous long-term care experience is preferred
• 2 years proven experience as an MDS Coordinator in a long-term care setting.
• Conduct and coordinate the Minimum Data Set (MDS) assessments for residents in accordance with federal and state regulations.
• Collaborate with interdisciplinary teams to gather information for comprehensive resident assessments.
• Ensure accuracy and completeness of MDS assessments to support resident care planning and regulatory compliance.
• Stay updated on changes in regulations related to MDS assessments and implement necessary adjustments.
• Actively participate in care planning meetings and contribute to the development of individualized resident care plans.
• Monitor and track resident progress, updating assessments as needed to reflect changes in health status.
• Work closely with nursing and administrative staff to facilitate accurate billing and reimbursement processes.
• Provide education and training to staff on MDS assessment processes and documentation requirements.
• LPN with MDS experience MAY be considered
