Complete Care at Brick is hiring a Registered Nurse-RN-MDS Coordinator to ensure quality outcomes, accurate assessments, and individualized care plans. You will work with a collaborative interdisciplinary team to drive excellence in resident care.

Qualifications

• Associate's or Bachelor's Degree in Nursing.
• Current, unrestricted New Jersey RN license.
• Experience in long-term care preferred.
• Experience as RN-MDS Coordinator or MDS assessment in skilled nursing preferred.
• Strong clinical assessment, communication, and organizational skills.
• Knowledge of federal and state MDS regulations.
• Ability to work collaboratively with interdisciplinary teams.

Responsibilities

• Coordinate and complete accurate MDS assessments per guidelines.
• Partner with nursing, therapy, social services, and other departments to develop care plans.
• Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.
• Monitor assessment schedules and update MDS documentation as conditions change.
• Participate in care plan meetings and contribute clinical expertise.
• Educate and support staff on MDS processes and regulatory updates.
• Work with leadership to maintain compliance and promote quality improvement.
• Help ensure Complete Care at Brick provides high level resident-centered care.

Registered Nurse-RN-MDS Coordinator Qualifications

• Associate's or Bachelor's Degree in Nursing
• Current, unrestricted New Jersey Registered Nurse (RN) license
• Previous long-term care experience preferred
• Experience as a Registered Nurse-RN-MDS Coordinator or MDS assessment experience in a skilled nursing setting preferred
• Strong clinical assessment, communication, and organizational skills
• Knowledge of federal and state MDS regulations
• Ability to work collaboratively with interdisciplinary teams

Registered Nurse-RN-MDS Coordinator Responsibilities

• Coordinate and complete accurate MDS assessments in accordance with federal and state guidelines.
• Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans.
• Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.
• Monitor assessment schedules and update MDS documentation as residents' conditions change.
• Participate in care plan meetings and contribute clinical expertise to improve resident outcomes.
• Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates.
• Work closely with leadership to maintain compliance and promote continuous quality improvement.
• Help ensure Complete Care at Brick continues to provide the highest level of resident-centered care.
