Job summary

Complete Care Regent in Hackensack, NJ is seeking an RN MDS Coordinator for a full-time role. You will monitor resident assessments, guide completion of MDS, and coordinate care plan conferences, ensuring timely and compliant documentation.

The ideal candidate will hold an active NJ RN license, possess knowledge of Case-Mix and Medicare PPS, be able to work independently or in a team, and have computer skills.

Qualifications

• Active NJ RN license required.
• Strong knowledge of Case-Mix and Medicare PPS processes.
• Ability to work independently or within a team.
• Computer literacy required.
• Previous MDS experience in LTC/SNF is strongly preferred.

Responsibilities

• Monitor and guide completion of resident assessments in a timely manner.
• Involvement and supervision of inputting MDS assessments.
• Transmit MDS assessments to the state and obtain receipt of validation.
• Schedule and facilitate care plan conferences on a quarterly and annual basis.
• Other duties as assigned by Director of Nursing and Administrator.

Compensation: $85K-$95K/ year (varies based on experience and licensure)
