Job summary

Complete Care at Shorrock Gardens in New Jersey seeks a Registered Nurse - MDS Coordinator to ensure quality outcomes, accurate assessments, and individualized care plans for our residents. You will collaborate with an interdisciplinary team to drive compliant, resident-centered care.

The ideal candidate holds an Associate's or Bachelor's in Nursing, an active NJ RN license, and experience in long-term care with MDS processes.

Qualifications

• Current, unrestricted New Jersey RN license.
• Experience as an RN-MDS Coordinator or MDS assessment in skilled nursing preferred.
• Prior long-term care experience preferred.
• Strong clinical assessment, communication, and organizational skills.
• Knowledge of federal and state MDS regulations.
• Ability to work collaboratively with interdisciplinary teams.

Responsibilities

• Coordinate and complete accurate MDS assessments per guidelines.
• Develop individualized resident care plans with interdisciplinary teams.
• Ensure documentation supports quality care, compliance, and reimbursement.
• Monitor MDS schedules and update documentation as conditions change.
• Participate in care plan meetings and share clinical expertise.
• Educate staff on MDS processes and regulatory updates.
• Support leadership to maintain compliance and quality improvement.
• Help ensure continued resident-centered care.

Registered Nurse-RN-MDS Coordinator Qualifications

• Associate's or Bachelor's Degree in Nursing

• Current, unrestricted New Jersey Registered Nurse (RN) license

• Previous long-term care experience preferred

• Experience as a Registered Nurse-RN-MDS Coordinator or MDS assessment experience in a skilled nursing setting preferred

• Strong clinical assessment, communication, and organizational skills

• Knowledge of federal and state MDS regulations

• Ability to work collaboratively with interdisciplinary teams

Registered Nurse-RN-MDS Coordinator Responsibilities

As a Registered Nurse-RN-MDS Coordinator, you will:

• Coordinate and complete accurate MDS assessments in accordance with federal and state guidelines.

• Partner with nursing, therapy, social services, and other departments to develop individualized resident care plans.

• Ensure documentation supports quality resident care, regulatory compliance, and reimbursement accuracy.

• Monitor assessment schedules and update MDS documentation as residents' conditions change.

• Participate in care plan meetings and contribute clinical expertise to improve resident outcomes.

• Educate and support clinical staff on MDS processes, documentation standards, and regulatory updates.

• Work closely with leadership to maintain compliance and promote continuous quality improvement.

• Help ensure Complete Care at Shorrock Gardens continues to provide the highest level of resident-centered care.
