Job summary

St. Joseph Of The Pines is seeking a full-time RN Manager/Clinical Care Coordinator to oversee resident-centered care in a Skilled Nursing Facility. Reporting to the Director of Nursing, you will coordinate clinical services, manage care plans, and collaborate with families and the interdisciplinary team to drive positive outcomes.

You will lead clinical care, ensure regulatory compliance, and support nursing staff across shifts in a mission-driven, equal-opportunity environment.

Qualifications

• Graduate of an accredited School of Nursing.
• ADN or BSN required.
• Current Registered Nurse License in North Carolina.
• Minimum one year of recent long-term care or skilled nursing experience.
• Strong clinical assessment, communication, and leadership skills.

Responsibilities

• Complete resident assessments and develop individualized care plans.
• Coordinate admissions, discharges, and ongoing clinical care.
• Monitor resident outcomes and update care plans as needed.
• Administer medications and treatments in accordance with established policies.
• Provide clinical leadership, delegation, and support to nursing staff.
• Collaborate with residents, families, providers, and care teams to ensure quality care and resident satisfaction.
