Job summary

The Health Center | St. Joseph of the Pines | Southern Pines, NC is seeking a full-time Registered Nurse (RN) Manager/Clinical Care Coordinator in a Skilled Nursing Facility.

You will coordinate clinical services for an assigned resident population and guide care planning under the direction of the Director of Nursing. Responsibilities include assessments, admissions/discharges, care plan updates, medication administration, and leadership of the nursing team to achieve positive clinical outcomes

Qualifications

• Graduate of an accredited School of Nursing.
• ADN or BSN required.
• Current Registered Nurse License in North Carolina.
• Minimum one year of recent long-term care or skilled nursing experience.
• Strong clinical assessment, communication, and leadership skills.

Responsibilities

• Complete resident assessments and develop individualized care plans.
• Coordinate admissions, discharges, and ongoing clinical care.
• Monitor resident outcomes and update care plans as needed.
• Administer medications and treatments in accordance with established policies.
• Provide clinical leadership, delegation, and support to nursing staff.
• Collaborate with residents, families, providers, and care teams to ensure quality care and resident satisfaction.
