Job summary

Personal Touch Home Health Care Services Inc. is seeking a compassionate part-time night shift Registered Nurse to provide quality care in a long-term care setting.

You will administer treatments, assess patient needs, coordinate care plans, and ensure accurate documentation while collaborating with the healthcare team to promote safety and resident well-being.

Qualifications

• RN license is required.
• Graduate from an accredited RN program.
• Current BLS certification is required.

Responsibilities

• Administer prescribed medical treatments and personal care to residents.
• Plan and evaluate resident care using the nursing process to achieve optimal outcomes.
• Perform comprehensive admission assessments and coordinate care plans.
• Gather, analyze, and document resident data across physical, psychosocial, spiritual, and educational domains.
• Communicate with residents, families, physicians, and healthcare team members to ensure continuity of care.
• Maintain accurate documentation reflecting the care plan and resident progress.
• Collaborate with team to meet resident needs and promote safety.
• Develop individualized care plans with residents and families, providing education.
• Supervise healthcare team members and assist in quality improvement initiatives.
• Contribute to discharge planning and provide information on available services.
• Perform other duties as assigned to support resident well-being.

Qualifications & Skills

• Graduate from an accredited RN program.
• Licensed as a Registered Nurse in the relevant state.
• Current Basic Life Support (BLS) certification.
