Job summary

Freeman Regional Health Services in Freeman, SD is seeking a compassionate Registered Nurse for Long Term Care. You will provide direct patient care, monitor residents, administer medications, and collaborate with CNAs and physicians to ensure coordinated, quality care.

The role is full-time, day shifts, with rotating weekends and holidays. You will be part of a team dedicated to respectful, person-centered care and adherence to Freeman CARE standards.

Qualifications

• RN license required (SD Board of Nursing).
• 1–3 years experience in clinical or long-term care preferred.
• Must be able to work hours specified and communicate effectively.

Responsibilities

• Regularly evaluate residents' physical and mental health, including vital signs and cognitive function.
• Dispense and monitor medications per physician orders.
• Maintain open communication with residents, families, physicians and care team for coordinated care.
• Observe changes in health status and promptly report to appropriate provider.
• Assess pain levels and administer prescribed pain meds.
• Provide emotional support and address psychosocial needs of residents.
• Maintain accurate resident records and logs.

Position Highlights Worker Type: Full Time - Benefit Eligible (working at least 72 hours per pay period) Work Shift: Day Shifts (6:00 a.m. to 6:30 p.m.) Rotating weekends/holidays

You can develop deep connections with residents and their families, getting to know their stories and personalities over time, while making a positive impact on their quality of life. By providing attentive care, you can significantly improve the daily lives of residents, making a noticeable difference.

What you will do:

• Regularly evaluating residents' physical and mental health, including vital signs, cognitive function, and overall well-being.
• Accurately dispensing and monitoring medications according to physician orders.
• Maintaining open communication with residents, families, physicians, and other healthcare team members to ensure coordinated care.
• Observing for any significant changes in resident health status and promptly reporting them to the appropriate healthcare provider.
• Assessing pain levels and administering pain medication as prescribed.
• Providing emotional support and addressing psychosocial needs of residents.
• Maintaining accurate and detailed resident records, including progress notes and medication administration logs.
• Recognizing subtle changes in resident behavior or health status due to the chronic nature of conditions.
• Collaborating effectively with Certified Nursing Assistants (CNAs) and other healthcare professionals to deliver comprehensive care.
• Comply with all aspects of resident rights including confidentiality and privacy.
• Lead, by example, following Freeman Regional Health Services CARE standards.

Required Education, License/Certification, or Work Experience:

• Registered Nurse (RN) - SD Board of Nursing 1-3 years experience working in a clinical or long term care environment, preferred.
