Job summary

Behavioral Health Network, Inc (BHN) is seeking an LPN to join the Respite team in Holyoke and Westfield. You will provide direct nursing services, perform assessments, and administer/verify medications while coordinating with providers for aftercare planning and documentation.

The role requires MA LPN licensure and a background in behavioral health, with a per-diem schedule offering opportunities across Western Massachusetts.

Qualifications

• High school diploma or GED.
• Minimum of 1 year of training in behavioral health and 1 year post-graduate nursing experience.
• Massachusetts LPN licensure required.
• Basic understanding of mental health issues and psychoeducational interventions.

Responsibilities

• Provide direct clinical and nursing services in the Respite program.
• Conduct intake nursing assessments and administer/ transcribe medications.
• Record progress with treatment and coordinate with providers for aftercare planning.
• Communicate with MAP staff, providers, pharmacy and others for treatment needs.
• Complete nursing assessments, progress notes, medication orders, and discharge summaries timely.
• Provide MAP competency training to staff on high-alert medications.
• Maintain professional boundaries in behavioral health interventions.

What You’ll Do

The Licensed Practical Nurse (LPN) for Respite provides direct clinical and nursing services to individuals served by the program. The LPN performs intake nursing assessments, administers, transcribes and verifies medications, records progress with treatment, consults providers, provides medication education and facilitates aftercare planning for individuals served.

• Consults regularly with Nurse Manager, providers, program staff for purpose of referral/ treatment planning, and medical interventions for individuals served.
• Performs comprehensive nursing assessments, including review of systems, diagnosis and mental status to treat or recommend treatment for needs that arise.
• Transcribes and administers medication to participants if MAP staff are not available, maintaining accurate MAP program documentation.
• Interacts with providers/pharmacy/Emergency Departments/significant others to assess and treat the individual’s needs and current situation.
• Completes all paperwork such as nursing assessment, progress notes, transcribing medication orders, and discharge summaries in a clear, timely manner to document treatment.
• Provides training to MAP-certified staff using the MAP Competency Evaluation Tool Forms for general knowledge and individual specific procedures for high-alert medications.
• Maintains appropriate boundaries for behavioral health interventions with individuals served.

Who You Are

• High School Diploma or GED (required)
• Minimum of 1 year of training working in behavioral health and 1 year post graduate experience in medical-surgical nursing required.
• Licensure as a Licensed Practical Nurse in the Commonwealth of Massachusetts required.
• Must have basic understanding of mental health issues and appropriate pharmaceutical and psychoeducational treatment interventions.
• Ability to provide high-quality community-based care to program participants, including information about recovery, health, wellness and medication education to individuals served.
