An established industry player in healthcare is seeking a compassionate Registered Nurse (RN) Hospice Case Manager to join their dedicated team. The Registered Nurse (RN) Hospice Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to foster a quality experience in accordance with the patient and family’s care plan and wishes. The RN Case Manager manages symptoms of the patient's disease and re-evaluates effectiveness of interventions and adjusts the care plan as needed. The RN Case Manager plans and manages the nursing component of the hospice caseload.
Location: Prince William County, VA - home care
Hours: Full-time: Monday-Friday 8:30am-5:00pm, occasional on call, 2 holidays per year

The Registered Nurse (RN) Hospice Case Manager provides, coordinates, teaches and supervises the nursing care of Hospice patients and families/caregivers. The RN Case Manager makes initial and ongoing nursing assessments, judgments and treatment decisions based on patient and family/caregiver needs. The RN Case Manager is responsible for supervising Licensed Practical Nurses, Certified Nursing Assistants, and Patient Care Volunteers. May be asked to rotate to other regions/departments.
Experience RequirementsMust have one year of staff nurse experience in a comparable setting/unit. Hospice and/or palliative care experience is strongly preferred.
Education RequirementsMust have a degree in Nursing from an accredited nursing program. A BSN degree is preferred.
Required Certificates and/or LicensesMust have licensure from the governing body for practice for the District of Columbia, State of Maryland, and/or the Commonwealth of Virginia, based on the state/jurisdiction of practice. Licensure from compact state is acceptable. Healthcare Provider BLS (CPR) is required to practice in the State of Maryland and the District of Columbia. A CHPN certification is a plus.
Driver's License
Must have a driver's license in good standing and access to a vehicle during working hours.
