This position is responsible for ensuring the continuity of care in both inpatient and outpatient settings, utilizing appropriate resources within established contracts and patients’ health plan benefits. It facilitates the continuum of patient care using basic nursing knowledge, experience, and skills to ensure appropriate resource utilization and quality outcomes. Care management functions are performed on-site or telephonically as needed.

This is a fully remote role where compact licensure is strongly preferred.

The schedule for this position includes some evening hours, with work until approximately 8 pm, based on member availability. For example: 11 am-8 pm or a split shift 8 am-12 pm and 4 pm-8 pm.
