Job summary

Centric Home Health and Hospice is seeking a compassionate Registered Nurse to provide in-home skilled nursing care. You will coordinate patient care with an interdisciplinary team, perform skilled procedures, and educate patients and caregivers on health needs and procedures.

The role requires a nursing degree, an active RN license, CPR certification, and at least 1 year of clinical nursing experience (home health preferred). Travel and flexible scheduling are involved.

Qualifications

• Graduate of an accredited diploma, associate, or bachelor nursing program.
• Current RN state license.
• At least 1 year of clinical nursing experience; home health preferred.
• Current CPR certification.
• Valid driver’s license with reliable transportation and auto insurance.
• Strong interpersonal and communication skills.

Responsibilities

• Conduct initial and ongoing patient assessments.
• Develop, implement, and update individualized plans of care.
• Provide direct skilled nursing care as ordered by physicians.
• Educate patients and caregivers on health needs and procedures.
• Communicate changes in patient condition to providers and care team.
• Complete clinical documentation accurately and timely.
• Participate in on-call rotation as assigned.
• Ensure compliance with infection control and agency policies.
• Uphold agency policies and professional standards.
