Job summary

Transitions Hospice - Illinois is seeking an RN Case Manager in Effingham, IL to coordinate care and guide the interdisciplinary team in providing compassionate end-of-life support.

You will conduct assessments, develop care plans, administer medications, maintain documentation, and educate families. A state RN license and hospice/home health experience are preferred.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited School of Nursing.
• 1+ year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs.
• Building and adjusting personalized care plans in collaboration with physicians and care teams.
• Administering medications and treatments per physician orders.
• Keeping clinical documentation accurate, timely, and meaningful.
• Serving as the communication bridge between families, physicians, and community health partners.
• Educating patients and caregivers so they feel confident and supported.
• Participating in on-call rotation per team policy.

8 AM-8 PM, alternating 4-day and 3-day weeks including weekends in rotation

What You'll Do
