Transitions Hospice in Illinois is seeking an RN Case Manager to serve patients at home, coordinating care and guiding the interdisciplinary team to deliver compassionate end-of-life support.
You will conduct comprehensive assessments, develop personalized care plans with physicians, administer treatments, document care, and educate families while participating in on-call rotation.
Coverage Territory: Effingham and surrounding areas
Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation
