Job summary

PruittHealth Hospice is seeking a Registered Nurse Case Manager to provide compassionate hospice care in patients' homes or care facilities. You will evaluate, plan, and coordinate nursing care according to individualized care plans while documenting accurately and following established policies.

Current RN licensure in the state is required. The role supports patient-focused care with competitive pay and comprehensive benefits in a dedicated hospice team.

Qualifications

• Current, active RN license in the state of practice.
• Experience in hospice or case management preferred.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan.
• Complete all documentation and records in a timely and accurate manner.
• Follow established policies and procedures as related to the scope of practice.
• Maintain continuity of patient care in a hospice setting.
