Job summary

PruittHealth Hospice is seeking a Registered Nurse Case Manager to deliver patient-centered hospice care in patients' homes or care facilities. You will evaluate and treat per individualized plan, and maintain accurate documentation.

This role emphasizes compassionate caregiving, adherence to policies, and coordination of services across disciplines. Active RN licensure in the practicing state is required, with preference for hospice experience.

Qualifications

• Current, active, and unrestricted RN licensure in the state of practice.

Responsibilities

• Complete evaluations, assessments, and treatments in accordance with policy and the individualized care plan
• Complete all documentation and records in a timely and accurate manner
• Follow established policies and procedures as related to the scope of practice
• Maintain continuity of patient care in a hospice setting
