Job summary

PruittHealth Hospice is seeking a Registered Nurse Case Manager to join our hospice team delivering patient-centered care in home or care facilities. You will evaluate patient needs, coordinate plans, and collaborate with interdisciplinary teams to ensure comfort and quality of life.
