Responsibilities

• Ensures that all activities performed align with the vision of Elara Caring’s leadership.
• Add key accountabilities here.
• Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
• Assesses assigned case load of home health care patients and families to identify needs.
• Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment.
• Assesses patient needs and incorporates data into the plan of care.
• Makes initial evaluations and re-evaluates nursing needs during visits.
• Communicates significant findings and changes in health condition to relevant team members.
• Revises the plan of care as needed and notifies team members.

Minimum Requirements

• Current, unrestricted state RN license
• 1+ year experience in a clinical care setting as a nurse
• Willing to travel within coverage area
• Dependable vehicle, valid driver’s license, and current auto insurance
• Willing to travel 50%
• Associates or Bachelor’s Degree in Nursing (preferred)
• Previous Home Health or Hospice experience (preferred)
• Experience with HomeCare HomeBase (preferred)

You will report to the Clinical Manager or Clinical Supervisor.
