Job summary

UPMC Home Health in New Castle is seeking a Full-Time Registered Nurse to provide in-home, one-on-one patient care across Pittsburgh-area communities. The schedule is Monday–Friday, 8:00 AM–4:00 PM, with on-call and weekend rotations, offering autonomy and strong interdisciplinary support.

You will deliver skilled nursing care, develop plans of care with physicians, educate patients and families, and document electronically using a provided laptop.

Qualifications

• Provide skilled nursing care in patients' homes.
• Develop and manage plans of care in collaboration with physicians and interdisciplinary partners.
• Provide patient and family education that promotes independence and recovery.
• Complete required documentation (OASIS, visit notes, orders) electronically using a provided laptop.
• Contribute to high-quality, patient-centered outcomes while maintaining regulatory standards.
