Job summary

UPMC Home Health is hiring a full-time Registered Nurse (RN) to support Beaver Falls, New Brighton, and surrounding areas. This role operates Monday through Friday, 8:00 AM–4:00 PM with on-call and weekend rotation, delivering one-on-one care in patients' homes and building meaningful patient relationships.

You will develop and manage plans of care in collaboration with physicians and the interdisciplinary team, educate patients and families, and complete essential documentation electronically

Qualifications

• Active RN license in Pennsylvania.
• Experience in home health or ambulatory care preferred.
• Strong clinical assessment and care planning skills.

Responsibilities

• Deliver skilled nursing care to patients in their homes across Beaver Falls and surrounding Pittsburgh-area communities.
• Develop and manage plans of care with physicians and interdisciplinary teams.
• Provide patient and family education to promote independence and recovery.
• Complete required documentation (OASIS, visit notes, orders) electronically using a provided laptop.
• Contribute to high-quality, patient-centered outcomes while maintaining regulatory standards.
