Job summary

BridgeWay Home Health is seeking an experienced Registered Nurse (RN) for full-time home health in Georgia. You will provide skilled nursing care in patients’ homes, develop care plans, and collaborate with physicians and families to deliver high-quality outcomes.

We value clinical autonomy, strong communication, and compassionate patient care within a supportive team. Compensation includes per-visit pay with mileage reimbursement and a comprehensive benefits package.

Qualifications

• Graduate of an accredited Registered Nursing program.
• Current, unrestricted RN license in the state of Georgia.
• Previous Home Health and OASIS experience required.
• Current CPR certification.
• Valid driver's license.
• Reliable transportation and current automobile insurance.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and documentation skills.
• Strong organization and time-management abilities.
• Ability to work independently while remaining connected to the clinical team.

Responsibilities

• Provide skilled nursing care to patients in their homes in coordination with physicians and family.
• Develop, implement, and update individualized plans of care.
• Perform skilled nursing interventions based on patient needs.
• Complete accurate and timely OASIS assessments and documentation.
• Educate patients and families about diseases, medications, treatments, and plan of care.
• Monitor patient progress and communicate changes in condition to the care team.
• Collaborate with interdisciplinary team to ensure quality outcomes.
• Promote patient safety and continuity of care.

What You'll Do

As a Home Health RN, you will provide skilled nursing care to patients in their homes while working closely with physicians, patients, families, and the interdisciplinary care team.

Your responsibilities will include:

• Conducting comprehensive patient assessments and reassessments
• Developing, implementing, and updating individualized Plans of Care
• Providing skilled nursing interventions based on each patient's clinical needs
• Completing accurate and timely OASIS assessments and clinical documentation
• Educating patients and families regarding disease processes, medications, treatments, and plan of care
• Monitoring patient progress and communicating changes in condition appropriately
• Collaborating with physicians and the interdisciplinary care team
• Promoting patient safety, quality outcomes, and continuity of care
• Providing compassionate, professional care that allows patients to remain safely at home
