Job summary

Bridgeway Home Health is seeking a full-time Registered Nurse (RN) for home health in Conyers, Georgia and surrounding areas. The RN will conduct one-on-one patient care, perform assessments, and collaborate on care plans with the team.

Flexible scheduling and competitive pay are offered, along with comprehensive benefits including medical, dental, vision, and 401k. Experience with clinical home health and OASIS is preferred.

Qualifications

• Graduate of an accredited Registered Nurse educational program.
• Current Registered Nurse (RN) license within the state of practice.
• Current CPR certification.
• Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills.
• Reliable transportation, valid driver’s license and current automobile insurance.

Responsibilities

• Conduct one-on-one patient care and patient/family teaching in the home setting.
• Perform patient assessments and collaborate with the care team to develop and implement a Plan of Care.
• Maintain timely and accurate documentation and communication with the clinical management team.
