Job summary

Bridgeway Home Health is seeking an experienced Registered Nurse (RN) for full-time home health visits in Cherokee County and surrounding areas, Canton, GA. The role offers per-visit compensation, mileage reimbursement, and full-time benefits.

You will provide skilled nursing care in patients' homes, develop and update care plans, complete OASIS assessments, educate families, and collaborate with the care team to ensure safe, high-quality outcomes.

Qualifications

• Graduate of an accredited Registered Nursing program
• Current, unrestricted RN license in the state of Georgia
• Previous Home Health and OASIS experience required
• Strong clinical assessment and critical-thinking skills
• Excellent communication and documentation skills
• Strong organization and time-management abilities
• Ability to work independently while remaining connected to the clinical team

Responsibilities

• Provide skilled nursing care to patients in their homes.
• Develop, implement, and update individualized Plans of Care.
• Perform OASIS assessments and accurate clinical documentation.
• Educate patients and families on disease processes, medications, and care plans.

What You'll Do

As a Home Health RN, you will provide skilled nursing care to patients in their homes while working closely with physicians, patients, families, and the interdisciplinary care team.

Your responsibilities will include:

• Conducting comprehensive patient assessments and reassessments
• Developing, implementing, and updating individualized Plans of Care
• Providing skilled nursing interventions based on each patient's clinical needs
• Completing accurate and timely OASIS assessments and clinical documentation
• Educating patients and families regarding disease processes, medications, treatments, and plan of care
• Monitoring patient progress and communicating changes in condition appropriately
• Collaborating with physicians and the interdisciplinary care team
• Promoting patient safety, quality outcomes, and continuity of care
• Providing compassionate, professional care that allows patients to remain safely at home
