Qualifications

• Graduate of an accredited LPN program.
• Current LPN license in the state of practice.
• Current CPR certification.
• Excellent verbal and written communication, organizational, collaboration, multi-tasking and time-management skills.
• Reliable transportation and valid driver's license.

Responsibilities

• Provide one-on-one patient care and family teaching in the home setting.
• Administer skilled nursing care per the plan of care and in the patient’s residence.
• Ensure timely documentation and collaborate with the interdisciplinary team.

Overview

As a home health LPN, you will be responsible for conducting one-on-one patient care and family teaching in the home setting. The LPN administers skilled nursing care to patients in their place of residence as prescribed and in accordance with the established plan of care. The LPN is also responsible for timely documentation and collaboration with the interdisciplinary team.

Requirements

• Graduate of an accredited Licensed Practical Nurse educational program
• Current Licensed Practical Nurse (LPN) license within the state of practice
• Current CPR certification
• Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
