Bridgeway Home Health is hiring a Licensed Practical Nurse (LPN) for full-time home health visits in Georgia. The LPN provides skilled nursing care in patients' homes, following the care plan and documenting thoroughly.

You will collaborate with an interdisciplinary team, educate families, and ensure high-quality, patient-centered care in the community setting.

Qualifications

• Graduate of an accredited LPN program.
• Current LPN license in state of practice.
• Current CPR certification.
• Excellent verbal and written communication.
• Reliable transportation and valid driver's license.

Responsibilities

Overview

As a home health LPN, you will be responsible for conducting one-on-one patient care and family teaching in the home setting. The LPN administers skilled nursing care to patients in their place of residence as prescribed and in accordance with the established plan of care. The LPN is also responsible for timely documentation and collaboration with the interdisciplinary team.

Requirements

• Graduate of an accredited Licensed Practical Nurse educational program
• Current Licensed Practical Nurse (LPN) license within the state of practice
• Current CPR certification
• Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
• Strong initiative, follow through, process orientation, attention-to-detail
• Reliable transportation, valid driver’s license and current automobile insurance
